That all of the information contained in this application is true and accurate. Any misrepresentations of fact may result in the removal of the adopted dog by Garden State German Shepherd Rescue, Inc.

That you understand that an adoption contract is part of the adoption process and will be enforced. Once a dog is adopted, if it is found to be neglected or abused, Garden State German Shepherd Rescue, Inc. reserves the right to reclaim the dog.
